Overview of Benefits:
Medical, Prescription Drug, and Vision Insurance
Effective: the first of the month following 30 days of employment.
Company sponsored plan: 100% of employee and 50% of eligible dependents’ premiums are paid by Rushforth.
Life Insurance/Accidental Death & Dismemberment (AD&D) Insurance
Effective: the first of the month following 30 days of employment.
Benefit Amount: $15,000 (benefits begin to reduce at age 65)
Company sponsored plan: 100% of employee premiums are paid by Rushforth.
Dental Insurance
Effective: the first of the month following 30 days of employment.
Company sponsored plan: 100% of employee, 50% of eligible dependents’ premiums are paid by Rushforth.
401(k) Plan
Waiting period: one year of employment (minimum 1,000 hours).
Employer Match: discretionary.
Vesting: 100% after 6 years of service.
Vacation Benefits

1 year 5 days (40 hours)
2—4 years 10 days (80 hours)
5 years or more 15 days (120 hours)
Sick Pay Benefits
5 sick days (40 hours) per calendar year.
Waiting period for new hires: 6 months of continuous service and the amount of sick days will be pro-rated based on the employee’s hire date.
Civic Leave Benefits
Immediate eligibility
The company will pay the difference between what the court pays and the employee's regular pay rate for a maximum of two weeks (80 hours).
Funeral Leave Benefits
Immediate eligibility
In the event of a death in an employee's immediate family, the employee will be allowed up to three days off with pay in order to assist with arrangements or to attend the funeral. Immediate family for this policy is considered: spouse, mother, father, daughter, son, brother, sister, grandparent, and in‑laws.
Educational Assistance Benefits
Waiting period: one year of continuous service.
With prior approval from the supervisor, the company will reimburse the eligible employee for 50% of the cost of a course - upon successful completion with a grade "C" or better. The course must be deemed directly job related by the company.
Gym Fees Benefits
Waiting period: one year of continuous service.
The company will reimburse the employee up to $35/month.
Three Bonus Programs
